This project will establish an antimicrobial stewardship program in a residential aged care facility, and will be implemented in partnership with Southern Cross Care (SA & NT) Incorporated.  It will trial the program in a single facility by establishing a team comprising a lead nurse, pharmacists, consultant physician in infectious diseases and a general practitioner to manage antimicrobial prescribing treatments, and will audit, educate and evaluate results from the surveillance of resistance and sensitivity patterns in cultures collected from residents.

A person centred volunteer program to improve the care and outcomes for older people with dementia and/or delirium in acute settings

This project aims to recruit and train volunteers to provide one-to-one practical assistance and emotional support for people with dementia and/or delirium admitted to rural and regional hospitals. Expanding a pilot volunteer program at Bega Hospital, the project will replicate the program across 7 Southern NSW Local Health District hospitals. ACSIHAG funds will support the rollout by employing project officers at each site to establish the program and support volunteers during implementation.  After the program is embedded, ongoing support and volunteer coordination will be assumed by the LHD. The Volunteer Program offers significant potential savings for the acute care setting and significant benefits to patients with dementia and their carers. Since this model of care is replicable, sustainable and low cost, it could be adapted across various healthcare settings and geographical environments to improve the care of people with dementia in hospital.
